NIH 398 and 2590 Forms Released
REVISED
FORMS PHS 398 AND PHS 2590 NOW AVAILABLE ON-LINE FROM NIH
On November 2, 2004, the National Institutes of Health
announced the release of revised PHS 398 and PHS 2590 forms
which are now available on-line. NIH encourages applicants
to access the instructions and forms via the Internet as
they provide valuable links to current policy documents and
allow easy navigation of the instructions. These revised
forms (rev. 9/04) can be utilized immediately.
If an application includes subawards, the revised
forms must be
used as of December 1, 2004 (see NIH revised application
policy). This announcement can be viewed at: http://grants.nih.gov/grants/guide/notice-files/NOT-OD-05-004.html.
However, if the
application does not include any subawards, the previous
version (rev. 5/01) will be accepted through May 9, 2005. All
applications received on or after May 10, 2005 must use the
new forms and applications using the previous version will
be returned.
NOTE: Currently the new forms are available as MS
Word or Portable Document Format (PDF). Both are
interactive. WARNING: Adobe Reader will not allow for
completed PDF forms to be saved -- this can only be done
utilizing Adobe Acrobat which must be purchased.

General
NIH Modular Grant Application Information
NIH Applications are to now
request direct costs in $25,000 modules, up to a total direct
cost request of $250,000 per year for all unsolicited new,
revised, and competing continuation R01, R03, R15, R21,
R41, and R43 grants and competing supplements, as well as
applications responding to RFAs for these mechanisms. Specific
grant mechanisms, e.g., R03, R15, R21, and Institute/Center
programs may define a particular number or range of modules
allowed.
Modular grant applications
are not to exceed $250,000 in total direct costs per year.
Unsolicited, investigator-initiated applications requesting
more than $250,000 in any year and applications for grant
activities not specified above will be required to follow
traditional application instructions and applicable NIH
policies.

SPECIAL NOTE:
The UCLA Office of Contract and Grant Administration
will no longer require a detailed budget for NIH applications
which fall under the Modular Grant criteria. The Principal
Investigator and departmental staff will be solely responsible
for ensuring that sufficient funds are requested and that
F&A (indirect) costs are accurately calculated for all
modular grant proposal submissions.

PHS 398 Application Kit
This kit is required for all applications
for new, revised, competing continuation, and supplemental
research grant, research training grant, and cooperative agreement
support except those which fall under the Modular Grant Process
(see above).
PHS
2590 Application Kit
For continued support of a PHS
grant, applications must be submitted on PHS 2590 forms
approximately 2 months before the begin date of the next
budget period. The Office of Policy for Extramural Research
Administration, Data Management Branch/EIS, will mail a
computer-generated Face Page (Form Page 1) and appropriate
mailing labels to the applicant organization approximately
4 months before the end of the current budget period.
