Industry Research Review Checklist (including Clinical Trials)

ACTIONS REQUIRED BY FACULTY MEMBER (THE STARTING POINT)
When
participation in private industry sponsored Clinical Trial
or other Research appears likely, the Faculty Member will
notify and submit to the Office of Contract & Grant
Administration a complete Proposal
Packet consisting of the following documents (Please
note that these are required)

Budget with approved
UCLA Indirect Cost Rate
Clinical Trial (click

26% Total Direct Cost
Research
On-Campus 54.5% Modified Total Direct Cost
Off-Campus 26% Modified Total Direct Cost
700-U Disclosure Form for PI
700
Supplement (If positive 700-U)
Copy of Protocol
Copy of IRB Informed
Consent (draft)
Sponsor contact name,
phone number, and e-mail address (as required on the
Goldenrod)
Copy of IRB or ARC Approval
(If required)
ACTIONS BY OFFICE OF CONTRACT & GRANT ADMINISTRATION
(OCGA)
Upon
submission and review of the Proposal Packet from the Faculty
Member the Industry Contract Officer will initiate:
Pre-Award
Process
Review and submit proposal to Sponsor
Contact Sponsor to initiate
contract negotiations
Provide Sponsor with Standard
UCLA Agreement
Obtain copy of previous
Agreement (If applicable)
Obtain copy of proposed
Company Contract
Review and propose revisions
to Contract
Negotiate final revisions
to Contract
Forward final Agreement
to Faculty Member
Faculty Member forwards
OCGA approved Agreement
OCGA signs Agreement and
forwards to Sponsor
Sponsor signs Agreement
returns to OCGA
NOTE: Faculty Member will submit IRB or ARC approval with
approved Agreement
Post Award Process
Prepare Draft Synopsis to obtain Acct-n-Fund.
Inform Department of Pending
Acct-n-Fund.
Prepare Final Award Synopsis
Distribute Synopsis and
Copy of Contract to Department.
Contract Maintenance
Amendments
Modifications
Addenda
Close Out
Department notifies OCGA of research completion including
total payment.
OCGA generates Final Synopsis
adjusting total award.
Initiate Policy 913 Fund
Extension.
NOTE:
UCLA Policy 913 Extensions apply to Clinical Trial and Fixed-Price
contracts ONLY!
